The safety and welfare of juniors at our championships is paramount. It is important that we have a note of all relevant contact details, illnesses, medical condition and other relevant health details relating to all junior competitors so that their best interests are addressed. Please complete the online parental consent form with our assurance that the information you provide will be treated as confidential.
